This Policy applies to all collected or submitted information received via the HPCTC website. HPCTC collects personally identifiable information and non-personally identifiable information.
Personally identifiable information (as used in this Policy) means any information that can be used to personally identify you, such as your name, address, telephone number, email address, etc. Personally identifiable information that is submitted to us is used to communicate directly with web site visitors or to assist HPCTC to better serve you as a visitor to our website, such as to respond to requests for information from you. Any information received is stored on a secure server operated by HPCTC or an agent of HPCTC performing services on our behalf. Non-personally identifiable information, in aggregate form, such as Internet Protocol (IP) addresses of our visitors, total number of visits to our website and to each page of our website, and the time spent on our website, is used to determine how visitors use our website and what we can do to make our site better for them. HIPAA
High Point Clinical Trials Center ("HPCTC") does not qualify as a "Covered Entity" under the Health Information Portability & Accountability Act of 1996. High Point Clinical Trials Center does however do business with health care providers and organizations that are considered "Covered Entities". When doing business with those providers and organizations, High Point Clinical Trials Center will adhere to the standard operating procedures for confidentiality of that covered entity. High Point Clinical Trials Center is dedicated to adhering to strict standards regarding the use and disclosure of your personal health information.

Although HPCTC takes reasonable efforts to protect the security the Site and Submissions, HPCTC is not responsible for any hacking attempt that may lead to the data being compromised.
You represent that you own all rights, titles, and interest, including all intellectual property rights, in Submissions that you submit to HPCTC.
Submissions that are submitted to other programs or Web sites owned or operated by HPCTC may be subject to additional terms and conditions of use and/or additional agreements (“Other Agreements”). Indemnification

Severability
